By consulting with Nutrition Edition, you agree to and understand the following:
I work within the guidelines and code of ethics set out by the Nutrition Society of NZ, and am a member and Registered Nutritionist under their professional accreditation.
All information in this questionnaire and discussions in any of our meetings is treated with the strictest of confidence. From time to time, I may consult with other practitioners that have specialist knowledge in order to provide you with the best service. If this happens, any information will remain anonymous.
Please ensure you are on time for your appointments so I don’t have to shorten your consultation time with me- this means I can provide the best service to all of my clients.
Advice given by an Associate Registered Nutritionist is not intended to replace advice from your medical practitioner. Any foods or supplements prescribed to you are not intended to replace any prescription medicine. If you require additional holistic, medical or dietetic support I refer onto a range of qualified and experienced practitioners to support your overall wellbeing.
Any advice given to you is intended for you personally and I cannot accept any responsibility for anyone you may choose to share this advice with.
I accept direct credit as payment and issue invoices within 24 hours of our consultation.
I ask that you please pay immediately; if you require some flexibility around this please let me know. Your Southern Cross Healthcare plan may cover my fees so check with them before you book.
If you need to cancel or change your appointment, I need a minimum 24 hours-notice please. Less than 24 hours notice will result in 50% of your consultation fee being charged.
A note on communication and hours of practice - My Practice hours are limited and I am often in consultations and therefore not responding to emails. On weekdays, I aim to respond within 24 hours.
